CaraVita Partners with Ceresti Health for Pilot Program

caregiverBeing a caregiver for a loved one is often challenging at times. It is important to be mindful of personal time and not overexert yourself when caring for someone. Inadequately monitoring your own time and needs can lead to the development of caregiver burnout. In addition to this outcome, it can be disheartening if your loved one has developed memory impairments and you may not fully understand what is happening to them.

How Does CaraVita Help?

Over the last few months, CaraVita Home Care has partnered with Ceresti Health to pilot their Dementia Education Program with families in the home care setting. We are always looking for new and innovative ways to put education and resources in the hands of those on the frontline of caregiving, and we are so grateful that Ceresti Health offered us the opportunity to run some of our families through their 10-week Dementia Education Program.

Ceresti Health created the first senior-friendly digital platform explicitly designed to empower family caregivers of aging adults. For each caregiver, Ceresti develops a personalized program tailored to their unique needs. The Ceresti program includes videos, tutorials, access to support, tools for engaging a loved one, reminders, and connects its users to other members of the care team. Online health coaching helps family caregivers understand the challenges of a patient with dementia and motivates a proactive approach to managing their loved one’s health.


Helping Families

Ceresti provides a senior-friendly tablet to family caregivers to help them learn about Alzheimer’s and dementia so that they are able to manage their loved one’s health inside of their own home proactively.  Family caregivers are matched with a dedicated Ceresti coach to guide their progress. The Ceresti coach empowers the caregiver to provide the best possible care for their loved one AND for themselves by offering ongoing support and resources. CaraVita had the opportunity to offer these services to some of our families, and the results were outstanding.

Whether it be a daughter, grandson, or spouse, family caregivers have a lot of responsibility. Even with the help of CaraVita’s in-home care, much of the burden of caretaking still rests on family members. The Ceresti team created a personalized plan for each of our five families during this pilot program, providing them with their own tablet and educational tools. Ultimately, the goal was to ease the burden of family caregivers by introducing them to this unique program. The benefits of which are:

  • Increased knowledge, skills and confidence
  • Aging in place with safety and dignity


What to Expect

If you are a caregiver for someone who is close to you with memory impairments, it can be easy to become frustrated. The first step to combatting negative emotions surrounding caregiving is to understand what you are dealing with entirely. The tables suggested by CaraVita, provided by Ceresti, educates families on a case-by-case basis on what they are dealing with. It helps relieve the burden of being misinformed and helps connect families to care. The tables can also remind a caregiver of specific medication application techniques and other essential processes.
